AROMATASE

A search on Medline for

aromatase insulin resistance

Troglitazone (a thiazolidinedione that improves insulin resistance) lowers elevated androgen concentrations in women with polycystic ovarian syndrome. Studies in porcine granulosa cells showed troglitazone did not affect aromatase activity but had direct effects on steroidogenesis. The drug specifically inhibited 3beta-HSD (3beta-hydroxysteroid dehydrogenase) activity, resulting in impaired progesterone production.

The typical high fat, low fibre Western diet may advance the onset of puberty; this is usually associated with earlier onset of hyperinsulinaemia which is a marker of increased breast cancer risk. Early menarche is reported to be associated with a raised oestradiol level persisting into early adult life. On the other hand, hyperinsulinaemia is commonly associated with abnormal aromatase activity in the ovaries. Life style changes reduce insulin levels in both children and adults, and "serial monitoring of insulin and sex steroid levels could be used to detect a metabolic-endocrine effect. "

Insulin has been shown to stimulate ovarian steroidogenesis; hyperinsulinaemia has been implicated in the raised androgen levels found in diseases associated with significant insulin resistance. Studies showed that in women with Diabetes 2, the ovaries have a reduced ability to convert androgen to oestrogen, probably due to a reduction of ovarian aromatase activity.
